Nonconformance report (NCR) template: free, and an AI agent that spots the repeat offenders

How to fill it in.
1 · Describe the nonconformance
Requirement vs. actual, with the part, lot and quantities. Mark where it was detected.
2 · Decide the disposition
Use as is, rework, repair, scrap, return or sort, with who approved it and the cost.
3 · Root cause, actions, closure
Why it happened, what's being done and who verified it worked.

One NCR is a problem. Five on the same part is a pattern.
NCRs pile up in a binder and nobody adds them up. The agent does: repeats by supplier, part and machine, cost of poor quality, and corrective actions that never got verified.
