5S audit checklist: the free template and an AI agent that checks it against the area
A one-page 5S audit checklist: 15 items, three per S, scored 0 to 4, with the finding and the actions. Fill it in by hand on the floor, snap a photo and your AI agent reads it. Snap the area too, and it checks whether what the sheet says shows up in the photo.
How to fill it in.
1 · Walk the area with the sheet
Score each item from 0 (very poor) to 4 (excellent) and write the finding even when the score is high: what and where.
2 · Take two photos
One of the sheet, whole and flat. One of the area, from the same spot every audit, so you can see the trend.
3 · Review and ask
Check what it read, confirm it and it goes into your plant memory. Then ask how each area is doing.

The area photo against the sheet: no more pencil-whipped audits.
The agent checks every item on the sheet against the photo of the area and tells you whether it matches, isn't visible, or the photo contradicts it. It never changes the auditor's score: it flags for review. And it lists what looks wrong in the photo but isn't on the sheet.
Matches
A 2 on floor cleanliness and an oil spill written down that shows in the photo: the sheet tells the truth.
Doesn't match
A 4 on set-in-order with three tools off the shadow board in plain sight: the agent flags it and shows where.
Not visible
Whether a standard exists or people know it doesn't show in a photo: it stays «not visible» and isn't counted as passed.

Can I change the checklist items?
Yes. If you change the form, the agent learns it from the first photo. Ours has a printed code so it's read the first time with no waiting.
Does the photo check change my score?
No. The score belongs to the auditor. The agent only flags the items where the photo says something else, so you can take a look.
Does it work for offices or warehouses?
Yes. The items are general; if your area needs different ones, adapt them and the agent will read them just the same.
