The problem

The most valuable knowledge in the plant is what nobody writes down.

The most valuable operational knowledge in the plant is what the shift lead carries in their head, and it is precisely what gets recorded nowhere. That filler two has been rejecting more since the reel change. That silo three smells odd. That the new operator has not quite got the hang of the CIP. None of it enters a system, because entering it would mean stopping, taking off the gloves, walking to a workstation and typing. So it gets told at handover, if there is time, and whatever is not told is lost. Three weeks later a complaint about a batch arrives and the information that would explain it no longer exists anywhere. There is a second problem, slower and more expensive: when that person rotates or retires, they leave with twenty years of plant in their head and nothing written down.
