The report already exists — and it dies in a group chat.

In a meat cutting room the incident report is given at second zero, but through the operator's own channel: the shift WhatsApp group. «Line 3 stopped 20 minutes ago», «batch XJ-22 is coming in with more fat than it should», «new blade on line 5, running 10 minutes of trial». That is gold-standard information, in real time, spoken by the person who is actually standing at the line.
